At Five Rivers, we believe prayer is one of the ways we carry one another, seek God together, and bring His hope into real life.


If you have a need, a burden, a celebration, or a situation you would like us to pray about, we would be honored to pray with you. Use the form below to share your request.

Because we value your privacy, you will be able to choose how your request is shared. You may share it publicly with our prayer wall, keep your name and contact information private, or send it confidentially to our prayer team and pastoral staff.


You are not alone. We would be grateful for the opportunity to pray with you.


Public Visibility Options:

  • Make Everything Public - Your request will be made public on our prayer wall and your contact info will be available.
  • Hide Name, Email, Phone - Your request will be be made public on the prayer wall, but it will be listed anonymously.
  • Hide My Request - Your request will be listed as "Confidential" and your contact info will not be available. 
  • Make Everything Private - Your request will not be listed on the prayer wall.

Staff Visibility Options:

  • Let everyone Know - Your Request and contact info will be made available to the prayer team and pastoral staff.
  • Hide My Name, Email and Phone - Your request will made available to the prayer team and staff but your contact info will be hidden.
